Authorizations: By signing below, each of the above listed merchant and merchant owner (individually and collectively, the "Applicant") certifies that Applicant is authorized to submit the above application on behalf of the above-named merchant (the "Application"). Applicant certifies that all information and documents submitted in connection with this Application are true, correct, and complete, and may be relied upon by BCA Capital Partners LLC and/or its affiliates, subsidiaries, and service providers (collectively, "BCA"). Applicant shall immediately notify BCA of any material change in its financial condition. If Applicant does not qualify with BCA, BCA has a large network of funding partners who may be able to help Applicant. In order to facilitate Applicant's access to the business capital Applicant needs, Applicant agrees that BCA may assign and deliver Applicant's Application (including Applicant's authorization and consent to obtain information from credit bureaus and other sources for purposes of qualification in connection with the commercial financing Application of the Applicant) to affiliated parties and/or BCA's small business funding network to determine whether Applicant qualifies for such third-party merchant cash advance services ("Financing"). Applicant also authorizes BCA to transmit and share this Application, along with any information obtained in connection with this Application, excluding consumer credit reports, with any or all of its' affiliates, subsidiaries, successors, assigns and designees, including third party lenders, service providers, contractors, and partners ("Recipients"). Applicant further authorizes BCA and all Recipients to request, receive, and review any consumer, personal, and business reports and other information about Applicant, including comprehensive credit histories or hard credit pulls (including Applicant's credit score), credit card processor statements, and bank statements, from one or more consumer reporting agencies, such as TransUnion, Experian, and Equifax, and from other credit bureaus, banks, creditors, and other third parties, for the purpose of obtaining Financing for the Applicant. Further, Applicant gives BCA, or an agent acting on its behalf, permission to call and/or send Applicant text/SMS messages at the phone numbers) provided with this Application, including Applicant's wireless number, using an automated telephone dialing system or other similar system for marketing purposes or any other purpose. Applicant expressly "opts in" to receive such communications for such purposes. Applicant understands that by signing below, Applicant is providing 'written instructions' to BCA and its Recipients authorizing BCA and its affiliates and subsidiaries to obtain information from Applicant's personal credit profile or other information from Experian. Applicant authorizes BCA to obtain such information in connection with the commercial financing Application of the Applicant.. Furthermore, Applicant hereby waives and releases any claims against BCA and its Recipients, and any information or service providers arising from any act or omission relating to the requesting, receiving, or release of the information obtained in connection with this Application. This authorization shall be valid for one hundred twenty (120) days unless revoked in writing by BCA or Applicant.

Notice: By providing Applicant's wireless phone number to BCA, Applicant agrees and acknowledges that BCA may call Applicant's landline and/or wireless phone number for any purpose, including marketing purposes, using all methods now known and discovered in the future, including, but not limited to, auto-dialers, artificial messages, pre-recorded messages, and general telemarketing practices. Applicant agrees that these calls may be regarding the products and/or services that Applicant has previously purchased and products and/or services that BCA may market to Applicant. Applicant acknowledges that this consent may be removed at Applicant's request but that until such consent is revoked, Applicant may receive calls from BCA at Applicant's wireless number.
